Guide to Apply Newborn baby passport in India ? Documents Checklist

The process to apply for Indian Passport for a new born baby is easier than you think. It is pretty much similar to the Indian passport renewal/ re-issue in India. The process is straightforward and there is NO need to pay to an agent or anyone for this. I did everything on my own, without seeking anyone’s help. I received my baby’s passport in about a little over a week via registered post. I applied it as normal processing mode ( NOT Tatkaal). Let’s look at the details. At the end there are users experiences and many new updates in comments, do check them out.

Minor Passport Spouse Name Endorsement Requirement

If either of the parents or both of them have passport, there is a requirement to have spouse name endorsed on the passport. Without having this, you will not be able to apply for passport. Endorsement means, on the pages with address and other info, you will see your spouse name as well, see below screenshot.

Spouse Name Endorsement Baby Passport India
Spouse Name Endorsement Baby Passport

The general process is to apply for Spouse Name Endorsement first and then apply for baby passport. But, there is a work around for this. You can apply for both Passport of the Baby and also Spouse name endorsement on the same day. Read How to apply for minor Passport without Spouse Name Endorsed Experience.

Now that you know the basic requirements, let’s look at the details step by step.

Step 1: Apply for Birth Certificate

Our baby was born in a little town in India and the first and foremost thing that I did was apply for a birth certificate at the municipality office. Depending on where your baby was born you would have to go the respective government body like Municipal office and apply for birth certificate.  Also, the hospital your baby was born in will ask you to fill out a form and you would need to mention all the details clearly on that, as that form will be sent to the municipal office or govt body.

It takes about two weeks to a month to get that. Nowadays everything is electronic. Depending on the state, you would have the state body give you a digitally signed birth certificate with an application number. I could check the details in the Govt website using the application number. You can submit your Passport copy and your spouse’s passport copy or Aadhar copy for spelling and proof.  Make sure your name, your spouse name and baby name are printed correctly, if not you will have issues. I had to re-apply for my birth certificate requesting corrections for the name as the address was wrong and they messed up the surname of my baby.  Eventually, I got the birth certificate of my baby in about 1.5 months.  This is the most important or the only document required I would say.

Step 2 : Register Account in Passport Seva Website, Complete Application

First thing you need to do is register and create an account at Passport Seva Website.  When you are registering a new account, you may not find the exact Passport Seva Kendra (PSK) center in the drop down as it may not be the major one and just a collection or appointment  center. In that case, you select the head of the state. For instance, you may not see Tirupati in the list of  Passport Office, when you register, you will have to Select Hyderabad.  It can be very misleading, but trust me, I did exactly the same thing by selecting the head office.

The process is pretty standard and ensure that all the spellings are correct. Make sure you give a valid email ID, as the link to activate your account and other status emails will be sent to the email address. Once you register you will get an email to activate your account, then you are set with registration.  You can check the guide to Applying Indian Passport Online, the basic steps on the website are same irrespective of the kind of passport.

You need to complete the application online, it is very much straight forward, you can download the PDF form and save and submit it or fill it online. Just make sure you complete the application online.

Step 3 : Walk-in with Application Receipt Printed (Check with PSK)

Some PSKs allow direct Walk-in appointments for some categories like Senior Citizens and Minors without prior appointment.  This always depends on the PSK and you will need to check with the PSK for this by calling them. In case, they allow direct walk-in appointment, you will need to take a print out of the Application Receipt (ARN) and carry that with you.

You can pay the fee online and take the appointment as well.  As you are applying for minor, you may do a walk-in as well and pay it there.  In my case, I paid the fee online and took appointment as written in the below step 4 as I did not know that I can walk-in.  One of our readers Raju (Thanks to Raju ) shared his experience that you can do walk-in. But, we have been reported by other readers like Suresh in Delhi that you cannot do walk-in directly and need to take appointment.  In any case, to avoid issues, I highly recommend getting an appointment, if you have time to avoid any confusions at PSK.

Step 4  : Pay and Book Appointment on Passport Seva Kendra Website

After you register, activate and login to your account and complete the application, you will be have options to book an appointment. Look at the left side navigation, you will have service section saying “View my saved/submitted applications”. Click on that and then you will get options to “pay and schedule appointment”.  You will need to do payment before you can book an appointment.

You can pay using challan or visa/master card. Only certain banks are listed like State Bank of India, if you want to use specific bank card. I recommend using any Visa or Mastercard of any bank and get through the payment, as it saves a lot of hassle. I used a visa card of some different bank and had no problems. You can just pay first and do NOT book, if you are not sure of the schedule. I first paid the fees and completed the payment process. I was waiting for the right dates to open for booking slot.

Usually dates would be available at about 4 to 5 days before the actual date of appointment. So, it is pretty clear, wait for the slots to open and book the date.  Also, there is no hassle in process, where you need wait at a certain time of the day for the slots to open and there is so many people booking at same time. With the payment process in place, it is very clear and there is no artificial demand created, so just book it at your convenient time, no need to pay some for getting slot.  Once your slot if booked using the system, take a print out the appointment receipt as you will need it for the entry at the Passport Seva Kendra ( PSK).  Read all the instructions clearly on the appointment letter as it has all the information you need.

Documents Checklist for Newborn Baby Passport in India

Assuming your situation is straightforward, with both parents alive and both are legally together, below are the Required documents List :

  1. Birth Certificate issued from the Govt Authority like Municipal office.Carry a copy of this.
  2. Proof of present address – usually parent’s passport with address is sufficient. If not, any utility bill, Aadhar Card, Gas connection, Election Voter ID, etc. with the address. We took copies of our passports ( mine and my wife’s) as it served as the address proof documents and other identification proof. Carry a copy of the address proof.
  3. Parents need to print and sign Annexure D from the passport Seva Kendra website
  4. One Passport Photo of the Baby with White Background.
  5. Printed copy of the PSK appointment receipt.

Optional documents:

My wife’s name was endorsed on my passport, so it was very straightforward. If not, it does not hurt to carry the Marriage certificate to avoid issues.

Common FAQs

I do not have spouse name endorsed on parents passport, can I apply for baby passport ?

You need spouse name to be endorsed on passport of at least one of the parents, if not, you cannot apply. But, you can do the spouse name endorsement and applying for baby passport on same day. Check How to apply for minor Passport without Spouse Name Endorsed Experience.

Do I need to take Baby to Passport Office ?

Yes, you need to take Baby to the passport office. They take fingerprints of the baby at the passport office.

My Spouse is abroad, cannot come to Passport Office. Can I apply on my own ?

In the past, as per passport application guidelines, they required either both parents to be present or get the Annexure D as sworn affidavit signed by Indian High commission abroad to apply for passport in India for baby. But, this has changed in recent years and there is NO need to have both the parents. If one parent is traveling or abroad, they can submit Annexure C and indicate the reason as traveling or abroad and get passport of the new born baby.

Why Spouse Name endorsement for getting Baby passport ?

This is to get passport for the baby without police verification.

How long does it take to get Baby Indian passport ?

This totally depends on the PSK and hard to say. But, if everything is right and no complications, you should get passport in about 10 to 15 days under normal processing.

It is always recommend you all to check the passport office website to verify, if any changes to the process or required documents.

Baby Passport Applying User Experience at  PSK Ghaziabad

Hello All,  I went to PSK Ghaziabad today for the passport of my 7 month old baby and would like to share the entire story :

Documents Required :
1) Self Attested Copies of Father+Mother Passport ( With Originals )
2) Self Attested Birth Certificate of Minor
3) Passport size photograph
4) Passport Receipt Form
5) Annexure D

First of all, there is no direct walkin service at PSK, Ghaziabad for Minors, so we had to book the appointment and went on time. We entered and on the very first counter, he checked our initial documents, placed them in the file and asked us to go inside. We went inside and waited for about 15 mins max and our name was announced. For Minors, PSK give a token number starting from “S” which is a special case, so everywhere your token number will be treated on priority. And thats what happened, just when we were finished from Counter A ( max 10 mins ), our token number blinked for Counter B and then in next 2 mins, for Counter C. We were out in max 30 minutes.
It was such a great service there and we were highly delighted. Thanks PSK and Thanks TCS for such a great process.

Did I miss anything? Any other thoughts to add?

Spouse name endorsement is mandatory and there is no exception. You will have to get either of yours passports re-issued with spouse name endorsement. The addresses and names does not matter. You will need to carry your current address proof. The good thing is that you can take appointment on same day for both of these and get it done. Read Spouse Name Not Endorsed Apply Minor Passport

  3. Hi Kumar,

I need to get mother’s name corrected (wrong spelling) in my daughter’s birth certificate. For proofs I have, my wife’s passport and marriage certificate with correct names endorsed.

Could you please let me know, what is the exact procedure that you had to follow to get your kid’s birth certificate fixed? Not finding exact details online, appreciate your help.

    • Actually it is not spell mistake, it’s wrong surname. In birth certificate they added my surname to my wife’s name (removed her maiden surname) where as all her proofs we have are with her maiden name. We are preferring to keep maiden name on the birth certificate.

      Reply
    • Ankit,
In our case, we had to go to the Municipal office and use a form that they had for name corrections. Check it here . It varies by state, I suggest you check with the local authorities where your daughter was born. Typically, after your kid is born, the hospital sends those details to Municipal office or city corporation, you can ask the hospital where they sent the details and work with the same entity for getting it corrected. It can be a little bit of work, but unfortunately, there is no quick fix online !

  4. Hi, I want to apply a passport for my 1 1/2 year old son. The problem is that my wife still has her maiden name in her passport which she did not change after marriage. Can I still go ahead and apply for my son’s passport or is it mandatory that my wife’s name should be endorsed in my passport or vice versa (in which case she will have to apply for a fresh passport re-issue with name changed)?

Not really. Address on the birth certificate does not matter. Nothing in specific. The article has the list and process, just follow the steps.

  6. Hi,
Normally we dont expand initials in the birth certificate right. Or Is it mandatory that the full name in the birth certificate should exactly match as we need in passport where we have to expand all initials.
As Birth certificate is the only document, i am confused.

    Reply
      • Thanks.
But when applying for passport, wont they match the name as it is in birth certificate? Won’t that be a problem then.

        Reply
        • Vishnu,
If you have it expanded on birth certificate, it is fine. If NOT, that is fine too. What matter is that the last name initial has to match. In India, it is quite common to have only initial in birth certificate. In fact, mine had the same situation and I had no issue. They will validate the initials.

So we have her birth certificate, my husband’s name in my passport, Annexure I, no need of Annexure G ( sworn affidavit by Indian embassy in Malaysia) as he would be present during the appointment. My question is, since he is a Malaysian does he need to bring anything else. Please clarify

    • Rachana,
The spouse name has to be endorsed in one of your passport. He will need to carry his original passport and any other identification, if he has from Malaysia like driving license (just backup, not required ). You will need to also print and sign Annexure H.

    • Var,
Well, It can be tricky as passport office only looks at the birth certificate. I suggest you get the birth certificate corrected. I had similar issue for my kid’s birth certificate where there were some spelling mistakes. I did not take any chance and got the birth certificate corrected as it can create future issues for visa sponsorships , etc due to name mismatch.

  10. H Kumar – First of al thank you for articulating the information clearly. I have a question. I and my wife have passport. Both the passports have same address and both have our names endorsed as spouse. I also have the birth certificate of my daughter. My question is for Annexure ‘H’ do we need sign on a normal paper do we need to have attested by Notary

    Reply
    • Hi Suresh,

Annexure H is required on a Normal Paper and signed by both parents. Since you have your spouse name endorsed on your Passport and have the Birth Certificate and above all stay at the same residence there is not issue at all for applying for passport for your child.

If the Child is less than 6 years then certain PSK have walk in facility for Children and the process is completed in hardly 1-2 hours max. In Mumbai this facility is available in MALAD and ANDHERI PSK daily from 9:00 to 10:30 AM. The Online condition for this walk in is that you should take an appointment and do not pay the fees, you just need to have the ARN number. The fees can be paid on the counter. The passport should approved with Police Verification if there was no issues in any of the parents Police Verification Report and should be delivered to you in 5-6 working days

At least one of you have to apply for re-issue of the passport to have spouse name endorsed, as it is mandatory to have the spouse name endorsed in passport of minor. Good thing is that, it can be done on the same day along with your kid’s passport, just do your first and then your kid’s. Read the recent experience Spouse Name Endorsed Minor Passport Experience

  14. Hi Everybody,

I am just summarizing the experience of passport for my son, with myself in abroad and my wife and son still in India. Even though passport site has detailed description, still we common people get many questions during applying, which are not well answered.

Before I start sharing, a request to all of the people is, please read all the thread instead of being lazy and hoping someone will answer. Every single person has some experience which may help you and your answer is already there.

    Ok
    Coming back to my scenario:
I was ready to fly abroad and both my wife and son has to apply for passport, with only myself having valid passport.

Hope the documents for minor are clear. I won’t repeat here.

First question: Can we apply for my son when I am in India, without my wife having Passport? Why we thought is, to get rid of Sworn affidavit work in abroad.

Passport office declined request. Saying Mother also MUST have a valid passport.

So, disappointed me flied. My wife consolidated all her documents which took sometime as valid address proof was missing. All those who worry in this case of not having an address proof, can get a resident certificate from Tahasildar. Even though this was not mentioned in passport site, they will consider this.

Now during this, I went to Consulate with passport (additional documents may be required based on country, so call them and enquire) and got the Sworn affidavit. They will have the template, you just fill in and make payment. Together, I also got my passport copy attested by them. This was asked by PSK specifically. Get this also done, may cost additional few bucks.

Now documents are ready. Question arises, if my wife and son can apply together? If yes, can she simply walk in with baby in the walk-in slot?

This was again declined by Passport office. No clue why!? I felt this is a bit illogical. They sent her back saying you first apply, get passport and then only son can apply. However, the person at enquiry had told the other way around. He had informed that its fine to apply for son if you have a file number for mother.

So, I took separate appointment for my wife, she went submitted. Got passport and went with her passport to apply for my son. Now both Passport in our hand.

Process was very quicker. Minor’s passport comes within a week. There is lots of improvement in the passport service, with this SMS service giving you quick update.

Even though some chaotic situations were there, still we got passport quickly. Thanks to PSK.

So definitely there would be some questions for which you don’t get sufficient answers, you can do any of the below:
    1. Directly walk to the Enquiry staff in PSK with all documents you have.
    2. Write to respective PSK email ID. But response is not very quick here.

All the very best people. Hope you/your kids get passport soon and you will be with your family soon.

    • Thanks Yuvi for sharing your experience ! Quite helpful. You are right, they can apply for baby passport on the same day as the mother’s with the application file number, no need to have two separate day slots. Recently, my sisters family just did that in Bangalore on same day. Not sure, why they declined at the PSK you went to…technically it should be done and I have readers confirm the same in Pune and my friend had done that in Hyderabad as well.

I have a question, I have to apply for my twin 9 months old babies. We need to provide information like… name, date of birth in User Registration page at passport website. In user registration, should I create only one user by my name or I should create two users for both the babies?

    2) My Wife Passport has my name Endorsed as Spouse on her passport, but address is not the current address.

    6) Marriage Certificate.
    7) Xerox copy of a Non Registered Rent Agreement.

Kindly let me know whether these documents are sufficient for my kids Passport and whether a Police Verification is required.

During My wife’s passport police verification, the report was adverse since I had not mentioned my permanent address but once we replied to the notice asking for change in Permanent address we got passport in 3 days. Will this impact the passport process?

    • Sainath,
I think you have all the documents and as one of the parents passport has the spouse name endorsed, it is all covered for passport issuance of minor without any police verification. You can submit the current address proofs for your baby. In my view, you have all of it covered and should get passport on time. The previous issue should not impact anyways. Just follow the process, you should have your passport within a week or 10 days.

Yes, you can do changes at the PSK while processing done at stage A inside PSK. Normal application is quite fast these days like 10 days or so, if everything is right. Tatkaal is a choice, but it has its own requirements.

No need of originals. Your husband needs to go to nearby Indian embassy and get a sworn-in affidavit of Annexure H , duly signed by the officials there and copy of passport signed by your husband. You need to carry the actual copies that he sends after the process abroad with you to PSK office.

It is highly recommended to carry original marriage certificate to PSK, just to avoid any issues. Yes, he need to sign on the photo copy and send it along with the sworn affidavit. Well, I do not have experience with DPC, cannot advise. Most of the current process is online and doing the PSK way is quite straightforward.

    • Let me answer:

      Steps:
      1. Fill in the form online and submit it. Passport site does not allow you to directly take appointment without submitting the form.
      2. Take appointment and go. Alternatively, you can directly walk-in without appointment if your PSK allows. Majority of PSKs allow this, but in a specified time slot. Ex: 9.30 to 11.00.
In any case, application receipt to be printed and carried with documents.

    • Please make sure of following things when you apply for minors passport without appointment:
      1) The walk in is only applicable for minors below 7 years only
      2) Just upload the form and do not try to pay when you are planning to walk in. If by mistake you happen to pay and book appointment then make sure you cancel the appointment and take printout of the receipt including the payment receipt.
      3) For new born, just make sure you carry passport size photo with white background.
      4) The walk in time slot is morning 9:00am to 10:30am

All above information is based on my experience at Pune passport office.

Please feel free to correct where needed.

      Reply
      • In case one parent is abroad, he needs to send the attested sworn affidavit from the consular office in that country. He/she need not be physically present at the PSK in that case.

    • Dear Akshit,

Yes you will need to take ur baby even she’s 2 days born.. The reason is they need to take the thumb impression on the generated application form over there which will be printed on her passport and they also need to have the either of the parents signature on the same form.
So its mandatory for each and every person to be physically present at the time of appointment.

    • Dear Sarvana,

I don’t think it will make any difference or trouble for that simply don’t check on any of I/We. When ur wife is at PSK ask the officials wat to do just carry it blank or filling the names and signatures only.

      Thanks

Having different addresses will not create any issues. I had similar situation. For your baby, use the address that is your current address and submit proof for the same.

    • Dear ashwini,

You have to apply through online for both of u and there is no option like minor or major but at the time of fee payment there would be difference.
For major, passport validity is for 10 years and in case of minor its 5 years.
You simply go to the website www.passportindia.gov.in and u will get all the information there.
And for any other assistance I am here to help you out.

      Thanks & Regards

U will need to produce and address proof for which u apply and for that there are 12 documents in passport address proof take any of those address proof the easy ones are

      1-bank account passbook
      2-Employer’s certificate on office stationary
3-u must have ur electronic bills or gas connection
      4-Rent agreement
      Etc.

Take them and if u don’t have bank account take it ASAP as joint account holder. It will help u for sure.

Check the website for the walk-in category of ur Regional Passport Office. If u fall in those category then u may be treated as walk-in there or u will need to take an appointment for both of u.

  30. Hi! Thanks for the valuable information. However, would like to get a little more on the following issues:

    1. Marriage Certificate is mandatory? For the purpose of attestation who should sign on the Marriage Certificate? Both parents?

    2. Annexure – H is mandatory?

    • Dear Bhargavi,

Birth certificate is not considered in address proof its only for the DOB and POB the applicant and make sure parents name are correct in that.
Don’t worry for the address in birth certificate.

  31. Hi..

    Need suggestion on below:

=> for infant’s passport application, i need to fill the online form, shall do the payment there itself and get dates. Now there are some questions regarding the documents to be carried:
    1. Birth certificate: Baby’s birth certificate is from Pune. We have it and no issues

    2. Parent’s name: Is it OK to apply for baby’s passport without having our marital status changed from Single to Married in our passport? Me and my husband have not changed our marital status in passport. Also, I have my maiden name (before marriage) in my passport. Do i need to change that also? We do have marriage certificate. Please suggest on this.

    3. Address proof: Will Light bill or gas connection on my husband’s name be ok for Pune’s address? Our current passport has different address – also different state.
Also, is address proof of both parents required? Please advise on this.

Are there any other documents that we need to carry or am i missing on anything. If so please advise. Thanks..

    • Dear Bhargavi,

You will need the both the parents address proof and the name should be the same as mentioned in ur Baby’s Birth Certificate.
Secondly let me clear you the parents passport is not mandatory in baby’s passport its only works in Police Verification.
And if you have changed ur name in birth certificate after marriage then u should carry ur marriage certificate and address proof with W/O.
Carry a white background photograph of ur child as well.

Finally make sure that there should not have any discrepancy in names to avoid any trouble.

      Thanks

    • Yeah u need to carry a passport size photograph of ur child with white background. And in case u r in abroad u will need to send a original affidavit its called a Consent (From parent residing abroad) Ur wife will be going the PSK with following docs.

1- passport of either or both parents
2-if spouse name not endorsed in passport then a address proof with correct name mentioned of spouse as mentioned in ur Consent
3- Birth Certificate of ur child issued from Birth and Death Registrar
4- Baby’s photograph with white background
5- Ann H ( u can get it thr also)
6- Consent from the parent residing abroad (attach ur passport copy if possible)

Make sure parents name and address proof is very important in case of a minor’s passport no discripency should be there if u don’t want any trouble.

I have all the documents and my wife is gong to PSK office to apply passport for my kid. The address in her passport and the address in my Baby birth certificate is different . is that fine? will that cause any trouble in applying for baby passport?

        Thanks,

        • No I don’t think it will make any trouble coz birth may be at different place. the address on which u apply for the passport must be there on the address proofs u r producing for that. Birth certificate have address which means parents address at the time of birth.

      1. Yes, you need to carry. Check your application receipt, it will have it as requirement.
      2. Both have to sign. If you are abroad, it has to be a Sworn Affidavit duly attested by the Indian Embassy/Consulate abroad.

  33. I want to apply my 3mth baby passport from mumbai.
My wife passport dose not carrying my name & surname and my name as husband not attached in her passport. As it was old.

Also my passport dosen’t carry my wife’s name.

So it is required that we have to include our names into each others passport??? or Marriage Certificate is enough as an evidence as a father-mother of baby.

    Kindly reply…!!!!!

    • Yatin, it is required to have the spouse’s name endorsed on at least one of your passport. The good thing is that you can apply for your baby passport and re-issue of your spouse’s passport with name endorsement. There will be police verification for the same. You will need to carry your marriage certificate to the passport office. Few months ago, a friend of mine did the same in parallel and got it done for their 3 month baby.

Should both the parents go for the appointment if I have Annexure H signed by both and have both the parents passport along with spouse name included.

    • As per recent guidelines by the MEA u both need to go for ur baby’s passport having the Ann H as well… Otherwise u will need a document named Ann G if accompanying single parent and its not easy to get Ann G. So if u both visit for ur baby’s passport it would be good for u.

I want to apply 1.5 month baby passport in chennai. Presently we are in kanchipuram but after 15 days i will move to Kuwait. In my passport doesn’t endorsed my wife name (spouse name) but In my wife passport endorsed my name. and also my passport address and my baby birth certificate address is different.
    I want to do?
It should compulsory spouse name include in passport or Marriage certificate is enough ?

Baby personally need to go passport office while document verification ?

    • Yuvarajan, If your wife’s passport has your name endorsed, that will be enough. Your passport Address and baby birth certificate address are not dependent on each other, is is fine. As your spouse already has your name, it is enough. Yes, baby needs to go to passport office personally.

I need your precious input for one of the situation I am facing.

We have a 6 months old baby. Only I have a valid passport with spouse name updated. Whereas my wife does not have a passport, yet to apply because of some document issues.

So I thought to apply passport for my baby and approached PSK Bangalore. The staff at enquiry told that both father and mother should have a valid passport. But atleast in passport seva in internet, nowhere I see this.

Can you please confirm which is correct?

    • Yes baby personally needs to go. They will take fingerprints
If any of the parent has spouse name endorsed, it is easier. Otherwise, there might be a police verification

      Reply
    • Sunil, It is mandatory to have the spouse name endorsed on either one of your passports. You need to apply for passport re-issue with that change to have the spouse name included, you need to show marriage certificate for that change. In fact, you can apply for this change in parallel with your baby’s passport application. Yes, baby need to be there on the day of the appointment. They take fingerprints. Also, if you are abroad, you need to go to the Indian high commission/ embassy and sign in-front of them on the respective annexure and present that same documents at the passport office.
