On page 1, you will see two sections: Primary Applicant Details and OFC Appointment Details. Below are the details of various fields and what they mean:
Primary Applicant Details
- Appointment(s) Made by: The person who booked the appointments.
- Number of Applicants: Total number of Applicants
- Passport Number: Passport Number of Applicant
- Visa Class: The Visa Class like B1/B2, F1, etc.
- Visa Category: Category of Visa such as Business/Tourism
- Visa Prioirty: Tells, if it is Interview Waiver / Dropbox
- DS-160 Confirmation Number: The DS-160 form confirmation number from CEAC website
OFC Appointment Details
OFC Stands for Offsite Faciliation Center, which is nothing but the VAC or Visa Application Center where you would submit the documents.
- Number of OFC Appointments: The total number of OFC Appointments. If more than one person are attending, it will be that number like 2 or 3
- OFC Appointment Number: Here it will list the number of appointment like 1, 2 or 3.
- Applicant Name: This will be the applicant who will be attending the above OFC appointment number. If more than on user, then this will be repeated later.
- Embassy/ Consulate/OFC : Location where you have selected for OFC for document submission. It does not mean you have to submit here, you can submit at any of the locations by paying a certain fee.
- Street Address: First part of the address related to the Embassy/Consulate/ OFC
- Street Address Cont.: Continuation of address of above
- City, Postal Code: Name of the city and Zip/Pin code.
- OFC Appointment Date: Date and time of the actual OFC Appointment to drop off documents.

On Page 2, there will be three sections “Document Delivery Information”, “MRV Fee Payments” and “Barcodes”. Below are the fields for each of these and what they mean

- Location name: Where the document or passport would be picked up
- Address 1 : Address of the pickup or delivery location
- Address 2: Address of the pick up or delivery location continued.
- City: city of the pick up location
- Post code: postal code of the pick up location
MRV Fee Payments
- Applicant Name: Name of the applicant for whom the fee was paid
- Receipt Number: MRV Fee receipt number
- Amount: Amount paid for the applicant appointment
Barcodes
- Applicant Name: Name of the applicant who is submitting documents
- DS-160 : The Barcode of the DS-160 and the same printed below the code
- UID: the UID of the applicant and the printed code below it.
