Do my spouse and kids have to fill separate DS-160 for H4 visa appointment?

Spouse and Kids are planning to go for H4 visa shortly. Kids are 7 and 2 years old. Do each member have to fill separate DS-160 or my spouse can include Kids in her DS-160? Do both kids 7 & 2 yr old have to appear in person for VISA interview? Kindly advice on other finer aspects to be taken care while preparing for H4 visa.

Eeveryone needs their own DS-160 form and needs to pay for the fees as well. The kids do not need to make personal appearance. You can search about interview waiver program which will be applicable in your case.
