Attending F1/F2 visa with my wife

I am applying for F1/F2 visa for me and my wife.

My question is,

In Dec 2010, while i was working in USA on h1b-visa I attended visa interview at chennai consulate and was issued 221g requesting additional documents.

Due to  my employers incompliance, i was unable to respond to that 221g at all.

Now since I am attending for a visa interview,

will there be questions related to my previous interview.

In DS forms, for the field asking for previous rejections, "were you rejected an visa " what am i supposed to fill in? Since i did not respond to 221g is it considered a rejection or as a withdrawal case?

Both are independent. If you receieved a form saying your visa is denied, then you would have to state it. Else, not required.
